Most people living with a life-limiting condition would prefer to stay living in their own home. For this reason over 80% of our care is delivered in the community.

Woking & Sam Beare Hospice is committed to providing care that is tailored to the individual at the time you need it but also in the place of your choice. We know that most of our patients prefer to be cared for in their own home, whether this be a private residence or in a care or nursing home, and so our focus is on working with our community healthcare colleagues to enable you to stay in your preferred place of care and to avoid unnecessary hospital admissions. We will always work with you to ensure that the care that we provide is focused on your wishes and we will support you in any decisions made about your care, both for now and the future.

No matter where we deliver your care, our team of healthcare professionals consists of highly trained and experienced Clinical Nurse Specialists [CNSs], specialist staff nurses and health care assistants, together with occupational therapists. The team also works closely with your GP and District Nurse to ensure that communication is shared and we work collaboratively, putting you at the heart of your own care.

Soon after we receive your referral your CNS will contact you to arrange to meet you and your carer. They will begin the process of planning and co-ordinating your ongoing care from local healthcare professionals and the specialist team at the hospice. We will provide assessment and will regularly monitor you and advice on the management of your symptoms. You will receive regular visits from the team appointed to deliver your individual care plan. This may also include medical review, therapies input, spiritual care, psychological and emotional support for you and your carers to help you cope with any fears or concerns. It is our aim to ensure you are able to remain in your preferred place of care for as long as is appropriate, while always safeguarding you and your carer.

Please always remember that it is your choice and you can change your mind at any time.

You will not be excluded from later being admitted as an in-patient to the ward [In-Patient Unit] if it had previously been your expressed wishes to remain in your own home. The facilities of the Wellbeing Centre are also available to you and to your carer and we can make arrangements for your travel to the hospice should this be required.
